We would like to have a Jira issue type that represents a SW Change Request. This will go through a workflow and at different stages the user will be required to attach documents, for example a copy of an email that grants approval for the change.
Rather than put these in attachments we would like to have dedicated fields for each type of document that is required. We are using Attach File Manager for Jira to create these fields.

When we come to audit a project we would like to export a PDF that contains the details of the project, this history, and embedded in the PDF a copy of the files that have been attached in the fields.
